Maximus operates this Site for the Indiana Family & Social Services Administration, Office of Medical Policy and Planning (“FSSA”). The site provides contact information. You provide information voluntarily when you phone or email Maximus. Maximus recommends that you do not email us personal information (such as a Social Security number or health information).
