Indiana PathWays for Aging (PathWays) is a Medicaid program for Hoosiers 60 years old and older. The PathWays program helps you get the care you need as you age. The PathWays health plans offer more choices for you to live independently while you get nursing facility level of care at home or in the community.

Medicaid offers health insurance to certain low-income families, people with disabilities, and older adults. The federal and state government fund it jointly.
Medicare is a federal health insurance program. It's for people 65 years old and older. Some people under age 65 who have certain disabilities or conditions may qualify for Medicare.
In managed care, one provider or organization coordinates all care for members. Some state Medicaid programs use managed care to give members high quality, cost-effective health care.
A Dual Special Needs Plan (D-SNP) is a type of Medicare Advantage health plan. It’s for people who have both Medicaid and Medicare (called “dual-eligible”).
The D-SNP gives people extra help because of income, disabilities, age, and/or health conditions. Your D-SNP works with your Medicaid health plan to coordinate your care.
